This artefact is a visual mapping tool designed to explore how health professionals coordinate care for trans and gender-diverse youth. Our approach engages participants in mapping key actors, information flows, and interactions, highlighting strengths, gaps, and challenges in multidisciplinary collaboration across clinics, families, schools, and community resources.
